A Rare Gastrointestinal Presentation in an HIV-Infected Patient
Question: A 27-year-old African American man recently diagnosed with human immunodeficiency virus (HIV) not on treatment presented with massive bleeding per rectum that required >7 U packed red blood cells transfusion. His initial hemoglobin was 2.2 g/dL that decreased from his baseline of 10 g/dL. After resuscitation, upper endoscopy and colonoscopy were performed. Colonoscopy showed multiple erythematous ulcerative lesion throughout the colon that was thought to be the source of bleeding (Figure A) biopsies were taken. Continue reading
